Hippotherapy is a treatment strategy used by a trained therapist that uses a horse’s specific, rhythmic, three-dimensional movement as a therapy tool, transmitted through the rider’s pelvis to stimulate balance reactions and postural responses similar to human walking. This is genuinely different from therapeutic or adaptive riding, which focuses more on recreational riding skills, even though the two are often conflated.

Hydrotherapy (aquatic therapy) uses water’s buoyancy to reduce effective body weight and joint loading, its resistance for gentle strengthening, and its warmth to help relax spastic muscles, making movement genuinely more accessible for children who find land-based exercise difficult, painful, or intimidating.

Here’s where honesty matters most: the research genuinely disagrees with itself.

A 2020 meta-analysis found real, favorable effects

Pooling 10 randomized trials and 452 children, this analysis found meaningful improvements on standard gross motor function measures, with effect sizes considered statistically large by conventional standards.

A more recent 2022 review reached a different conclusion

Published in a leading child neurology journal, this systematic review and meta-analysis found that adding hippotherapy to standard physical therapy produced the same result as physical therapy alone, while specifically flagging that the underlying studies had low methodological quality.

Neither finding cancels the other out

Together, they suggest real promise alongside real uncertainty. The mechanism is biologically plausible and some trials show genuine benefit; a more rigorous, more recent look at the same evidence base found it wasn’t clearly better than standard therapy alone, and flagged the studies themselves as not yet strong enough to be fully confident either way.

Multiple independent studies have found genuinely positive effects: improved gross motor function, better aquatic-specific function, enhanced social interaction, and improved physical quality of life among children with CP. The caveat, repeated across several of these studies specifically, is that exact dosage, frequency, and which functional levels benefit most remain genuinely underspecified in the current evidence. Researchers themselves keep calling for more work on optimal intervention protocols, not just whether the therapy works at all.

A useful way to calibrate expectations

A comprehensive review of CP treatment evidence found that only a small number of approaches currently have genuinely high-level supporting evidence, specifically bimanual therapy and constraint-induced movement therapy for hand function. Hippotherapy and hydrotherapy were explicitly categorized as therapies that might complement task or goal-oriented physical therapy programs, not approaches carrying the same strength of evidence behind them.

This isn’t a reason to dismiss either therapy. It’s a genuinely useful way to set realistic expectations and, if resources are limited, to think clearly about sequencing.
